ESSENTIAL Wellness Program

The ESSENTIAL Plus program fee is automatically charged to your payment method monthly on the same day of the month of your initial enrollment. If a payment is declined, you have 30 days to update your payment method. If not received within 30 days, ESSENTIAL Plus members will be transitioned to the ESSENTIAL Base pay-as-you-go structure.

The ESSENTIAL Base administrative services fee is charged to your payment method on file at the time of your visit or when specific administrative services (such as complex form completion) are requested.

The ESSENTIAL Plus program is $29 a month. The ESSENTIAL Base program is $49 per visit/occurrence.

Health insurance plans have complex criteria for what is and is not a covered service. Many of the administrative and digital services patients appreciate—such as insurance claim advocacy, secure portal infrastructure, and complex form coordination—are not covered by insurance. Instead of charging separate fees for these clerical tasks, the monthly fee provides the non-clinical infrastructure necessary to support these enhanced administrative amenities.

If you elect not to enroll in the ESSENTIAL Plus program, our team will provide the ESSENTIAL Base agreement for your review and signature at your next visit. The non-covered administrative fees for the Base option are $49 per occurrence. This pay-as-you-go choice is often best for patients who visit Health Moves once or twice a year and prefer to pay for administrative amenities—such as portal access and form coordination—only when they are utilized.

We limit the number of spots per provider to 200–250 to ensure we can continue to provide exceptional care, including reserved time each day for urgent issues.

Yes! You can cancel your enrollment at any time by filling out our Cancellation Form. Please allow 7–10 business days for processing. Upon cancellation, any unearned portion of your current month’s fee will be prorated and refunded.

Administrative benefits will terminate at the end of the month in which you cancel. Please note that if you choose to re-join the ESSENTIAL Plus program later, a $150 rejoining fee will apply, and enrollment is subject to provider availability.

Yes! You can transition from the ESSENTIAL Plus monthly program to the ESSENTIAL Base (pay-as-you-go) administrative structure at any time. To request this change, please contact our office so we can provide the updated agreement through your patient portal.

Furthermore, the practice limits the number of patients per doctor to maintain our high standard of administrative support; therefore, a spot with your specific provider may not be available if you attempt to re-join the Plus program in the future.

To keep our billing as simple and transparent as possible, we offer a straightforward monthly subscription for the ESSENTIAL Plus program. This allows you to access all our non-clinical administrative amenities and care coordination infrastructure for a consistent monthly fee of $29, with no long-term commitment. Patients selecting the self-pay option are not required to enroll in the ESSENTIAL Plus or ESSENTIAL Base programs. This includes self-pay services and services with Health Moves Consulting providers (Drs. Yarnell and Altman). These programs are elective and are specifically designed to provide enhanced administrative coordination and digital infrastructure for our insurance-based care model; however, self-pay patients are welcome to enroll in either program if they desire the additional administrative support and digital amenities

Yes! You have access to all the providers at Health Moves who offer a range of specialties and procedures.

We do not offer family or partner discounts at this time. The ESSENTIAL Wellness Program is structured at a per-individual rate to ensure that each patient receives dedicated administrative coordination, secure portal access, and personalized insurance advocacy tailored to their specific healthcare needs
